- 2007 ARCC Grant Application

- GAPS Form
- The GAPS (Gap Assistance Pharmacy Program for Seniors) form is for SC residents only. It is for seniors 65 or older who are Medicare eligible and have a gross household income below 200 % of the Federal Poverty Level and who need help "filling the gap" with their Medicare prescription drug plan.
- Hunting and Fishing License
- This links directly to the SC Department of Natural Resources (small 90KB pdf file). This is a lifetime license application. The fishing license is free for the disabled and those 64 and older
- Handicapped License Plate/Placard

- Medicaid - Long Term Care
- This form is for adults living at home who need help to pay for health care services to stay in their home. It also is for someone who is living in a nursing home or is applying to live in a nursing home and needs help to pay for the nursing home care.
